The Secret Surgery That Violates International Law
On May 15, 2026, three days before completing one year of arbitrary detention, Ruth López was secretly transferred from Izalco prison to a hospital in San Salvador to undergo a quadrantectomy — surgery to remove a tumor in one of her breasts. The regime of Nayib Bukele performed the procedure without notifying her family, without informing her lawyers, and without allowing anyone to visit her in the hospital.
The information was revealed by investigative journalist Héctor Silva Ávalos (@HsilvAvalos), who published on his X account (formerly Twitter) on May 19:
"Last weekend, Ruth López underwent a quadrantectomy, which is a surgery to remove a tumor in one of her breasts. The family was not warned and the government did not allow anyone to visit her in the hospital, where she only stayed a few hours."
Ruth was taken to the hospital on Friday, May 15, and returned to prison the next day. Everything in secret.
Violation of the UN Mandela Rules
The case constitutes a direct violation of the United Nations Standard Minimum Rules for the Treatment of Prisoners, known as the Mandela Rules. Specifically, Rule 68 states:
"Every prisoner shall have the right to immediately inform his or her family, or any other person designated as a contact, of his or her imprisonment, transfer to another facility, and any serious illness or injury."
The Bukele regime violated this rule in multiple ways:
1. It did not inform the family about the transfer to the hospital.
2. It did not inform the family about the scheduled surgery.
3. It did not inform the family about the medical diagnosis (the tumor).
4. It denied information to family members when they went to the hospital to inquire.
5. It kept Ruth incommunicado during and after the surgical procedure.
Additionally, Rule 24 of the Mandela Rules establishes that prisoners must have access to health services of the same quality as those available to the general community, and that transfers for medical care must be carried out with due transparency and notification to family members.
Cristosal's Response
Cristosal, the organization where Ruth worked as director of the Anti-Corruption Unit, issued a statement on May 19 denouncing the facts:
"Last weekend, relatives of Ruth López learned that she was admitted and operated on at a hospital in San Salvador. Family members were not previously notified about her transfer, the reason for the medical intervention, or her current health status."
The organization demanded:
1. That the family be immediately informed of her health status.
2. That family members and legal representatives have urgent access to her medical records.
3. That they be allowed to see her in person to verify her condition.
4. Her immediate release to guarantee her right to health and allow her to be accompanied by her family.
Cristosal recalled that the IACHR had requested in September 2025 that Ruth's health be monitored and that the possibility of her undergoing the process while free be examined. The Salvadoran State has systematically failed to comply with this recommendation.
Why the Secret Operation Is So Serious
The surgery without family notification is not merely a bureaucratic violation. It has profound implications:
Medical risk without family oversight: Every surgical intervention carries risks of complications, adverse reactions to anesthesia, infections, and the need for postoperative care. Ruth's family has the right to be informed and to participate in decisions about her health.
Lack of transparency about the diagnosis: It is known that she underwent a quadrantectomy to remove a breast tumor, but it is unknown whether the tumor was benign or malignant, whether additional treatments (chemotherapy, radiotherapy) are required, and what the actual prognosis is.
Impossibility of verifying the treatment received: By not allowing visits or access to medical records, there is no way to verify that Ruth received adequate medical care or whether she was subjected to undignified conditions during the procedure.
Pattern of opacity: The secret surgery adds to a consistent pattern of due process violations: 40 hours of initial forced disappearance, total incommunicado detention, changing charges without evidence, secret hearings, and systematically ignoring IACHR orders.
The Mandela Rules: A Standard Bukele Ignores
The Mandela Rules (adopted by the UN General Assembly in 2015 and named in honor of Nelson Mandela) represent the internationally accepted standard for the treatment of persons deprived of liberty. El Salvador is a signatory to the international treaties that incorporate these rules.
Other rules the Bukele regime has systematically violated in Ruth's case:
- Rule 58: Prisons must guarantee continuous, quality medical care.
- Rule 60: Prisoners must be transferred to hospitals when specialized care is required.
- Rule 61: Transfers must be made with the knowledge and authorization of competent authorities.
- Rule 19: The right to receive regular visits from family members.
- Rule 37: Every detained person has the right to notify their family of their detention.
What Does This Mean for the Future?
Ruth López's secret operation occurred in a context where the Salvadoran State, under the state of exception, has accumulated a documented history of human rights violations including:
- More than 90,000 mass detentions since 2022.
- Torture and enforced disappearances documented by international organizations.
- Secret prisoner transfers such as the agreement with Trump to send 252 people to CECOT.
Ruth's case — a lawyer who dedicated her career to defending human rights and fighting corruption — is the most visible example of a system that uses the judicial apparatus to silence those who dare to denounce power.
The international community has condemned the secret operation and demands her immediate release. But as long as the Bukele regime continues to operate without checks or supervision, Ruth's life — and the lives of thousands of political prisoners in El Salvador — remains in danger.
